All Guild members and associate members who have been members in good standing for at least one year are eligible for Guild life insurance benefits through Amalgamated Life.
If you haven't already signed up to name a beneficiary -- who would receive $10,000 upon your death -- you should! If you're unsure whether you have already named a beneficiary or you want to change your beneficiary, resubmit the form below. Your most recently completed application will supercede all previous applications.
